MCCoA’s outreach staff serve as advocates and problem-solvers for older adults and their families who aren’t sure where to turn. They listen to your situation, explain available options, and help you understand everything from Medicare plans to local support programs so you can make informed decisions with confidence.

Outreach workers can meet with you at our center or, when needed, visit seniors in their homes to review needs and complete applications. They assist with benefits counseling, referrals to community services, help with paperwork and forms, and family consultations to talk through care plans. In partnership with organizations like the Alzheimer’s Association, they also help coordinate care consultations and connect caregivers to support groups and specialized resources.

    Outreach support is available to older adults in Mercer County, this includes seniors living in Celina, Coldwater, St. Henry, Rockford, Fort Recovery, and surrounding rural communities, as well as family members and caregivers who are helping a senior navigate care decisions. There is no fee for this service; it is supported through community funding and the county levy. Whether your income is limited or comfortable, you can receive the same compassionate guidance. Our goal is to ensure no senior or family misses out on help simply because they don’t know where to look.
