Outreach Services

Definition and Purpose
Outreach workers are knowledgeable about many resources and programs available to seniors.  Appointments for in-office consultations are offered, as well as in-home visits for those unable to come to the office.

Many programs are available to all persons age 60 an older, while some do have income and/or asset limitations and other guidelines for eligibility.

Please call the office at 419-586-1644 to schedule a consultation.  A Mercer County Council on Aging outreach worker will meet with client(s) either in-office, or in-home to complete an individual assessment to help determine eligibility for each service.
